Derby City Council and Derbyshire County Council would like to invite you to complete a questionnaire for system partners who work with families with young children.
They are interested in better understanding and exploring the need for emergency provision of infant formula for families experiencing financial crises in both Derbyshire and Derby City.
Derby City and Derbyshire County Council are particularly interested in infants under 12 months of age, as breastmilk and formula milk (not cows milk) are the primary source of nutrients for this age group. They recognise that some families you work with may require support to feed older children, but for the purpose of this work, please try to filter your responses for families with children under 12 months of age only.
Why are we doing this?
There is currently a lack of data on this topic for both Derbyshire and Derby City.

Anyone who works or volunteers with families with children under 12 months in Derbyshire or Derby City
