Public Health would like your views on pharmacy services in Derby and Derbyshire. These services can be from your high street pharmacy, online, mail order or GP surgery.
Taking part in this survey will help Public Health to understand how local people use these services and their experience of them.
Your feedback will help Derby and Derbyshire Health and Wellbeing Boards produce a Pharmaceutical Needs Assessment (PNA) later this year. The PNA helps NHS England, local authorities and the local NHS to help plan and deliver local pharmaceutical services.
